Day Treatment Program

The #935 Self-Contained Program partners with Lakeland Mental Health to provide a therapeutic educational environment for children with behavioral and mental health needs; learning and implementing the skills necessary to become successful in the mainstream school, home, and community settings.  Families, supportive agencies, and school districts work closely to provide a comprehensive approach to providing the best educational program for each individual child. 

Determination of Student Eligibility:

- Must meet eligibility in one of the following educational categories: Emotional Behavioral Disorder or Other Health Disabilities.
- Need the intensity level of day treatment as identified in the student’s diagnostic assessment (DA). 
- The DA must be completed by a mental health professional or clinical trainee within one year of the initial start date of the program. 
- The DA must document children’s therapeutic services and supports as medically necessary to address an identified disability, functional impairment, and the individual’s needs and goals.
- Students admitted to children’s day treatment must be in need of and have the capacity to understand and benefit from the rehabilitative nature, structured setting and therapeutic components of the program’s psychotherapy and skills activities.
- Must be a resident or open-enrolled to a member district of #935 (Ashby, Battle Lake, Breckenridge, Campbell-Tintah, Pelican Rapids, Rothsay, or Underwood).

Process for Admission Consideration:

1. Sustained Restrictive Programming: with consultation from #935 Administrators. The referring district is prepared to implement a full time self contained program before or after student placement in #935 program.
2. Self-Contained Application: is completed. The student has met criteria for EBD or OHD.
3. Screening Team: Consists of #935 Administration and a Lakeland Mental Health Professional. Screenings will be scheduled within a week of receiving the application. 
4. Acceptance into the Program: if it is deemed appropriate for the student to start in the program, there will be a meeting between parent(s)/guardian(s), referring district representation, appropriate administrators, and self-contained day-treatment staff.
a. The referring district will write a new IEP with input from #935 staff.  The IEP will be presented by the home district with a Prior Written Notice to parents for a signature. Signature is required to start in the program.
5. Enrollment to the Program: enrollment paperwork for District #935 must be completed and submitted. 
a. After enrollment paperwork is submitted & there is a signed IEP, a start date will be communicated and transportation will be arranged with the referring district.
