Please STAPLE all verifications of experiences to this cover sheet – certificate of participation, agenda/schedule of experience, copy of grade slip or transcript, letter of involvement signed by supervisor, or other. Once all verification has been STAPLED, please number each piece of verification (1, 2, 3, etc.).
